• Applications of Aromatherapy in the Treatment of Traumatic Dissociation: A paper written for partial fulfillment of Clinical Aromatherapy Level 1 at Aroma Apothecary Healing Arts Academy.

• COVID-19 Waiver: This is the prescreening questionnaire and waiver for treatment during the COVID-19 pandemic.  A copy of this will be signed, in office, at your appointment time.  Please feel free to download and read in advance.

• Client Intake Form: While we normally use a digital intake form, there are cases where this intake form is preferred.  If you were asked to download and bring a physical intake form with you to your first appointment, this is what you are looking for.

• Minor Consent and Release Form: If you are the parent or legal guardian of a minor seeking massage therapy, you will need to read and sign this form in office.  If you would like to read it ahead of time, you may download it here.

• SoulFire Healing Arts Brochure: Overall brochure outlining the company vision, listing services offered and providing contact information.

• Trauma Touch Therapy™ Brochure: A brochure discussing both trauma and Trauma Touch Therapy™.  Contact information is also provided.
